NMU: Add missing licenses and copyright statements. (Closes: #1019273)

This is the patch for the NMU I made for 2.5.6+nmu1.

Since we're doing small and targeted fixes, I worked from the latest archive version instead of the git HEAD. This means I didn't upload this commit: f27de33f

Not sure how you'll want to integrate this? Happy to make changes if needed.
