self service: request test form forgets values when validation fails

Steps to reproduce:

1. Log in
2. click "Request test"
3. type a package name, and click submit.
4. notice that it complains that you need to select an architecture, but the package name field comes empty, i.e. it does not remember the value you had just typed. the same happens with the other fields.
