qa workflow: Implement fail_on parameter

For regression tracking (#791), the blueprint requires the qa workflow to gain a new fail_on parameter, described in Handling of fail_on.

Depends on #908 (closed).

To upload designs, you'll need to enable LFS and have an admin enable hashed storage. More information
