Mostly from Sean Whitton. Also revert some unrelated informative changes, leaving only clarity and formatting fixes. Drop the sample minimal debian/rules file, leaving only a reference to the dh documentation. Don't stress a new packaging helper to make it more obvious that cdbs is one of the expected exceptions to this advice.

Document dh as the recommended default approach to implementing debian/rules and provide some basic information about it. Include the basic dh debian/rules template and mention that it implements the default targets via a wildcard rule. Minorly clean up the language in the first two paragraphs of the debian/rules section.

The policy says in section 9.1.1 all files and directories must comply with Filesystem Hierarchy Standard (FHS) 3.0. Later in section 9.1.2, the references to FHS' section numbers were pointing to sections apparently only sensible for an older FHS 2.3 document. This fixes those references to the new numbers found in the FHS 3.0 document. See: https://bugs.debian.org/922654
