Thanks, Niels Thykier.

Some footnotes weren't replaced with [#], which prevented finding
the relevant footnote.  There were also some newline issues in some
of the conversion of description lists.

Clears up the last warning, at the cost of not listing example
headers in the cross-reference (which should be fine).

The long list of values for a description list was confusing
Sphinx and producing a warning, and was difficult to read.  Reformat
and move the list of supported VCSes into a separate bullet list.

globally convert to used parsed-literal blocks, but also some
italic/emphasis recovery for "variables" in 6.6 and and line
continuations.

Avoid nesting Appendix under the Legal Notice heading and move
the Legal Notice to the bottom.
