Split the git style descriptions into separate form fields

New request form: I don't think this line is clear: "Describe the request in a sentence or two. First line is the subject (like a git commit).". The UX (gray, at the bottom so you only see it once you'd finished typing) makes it easy to miss. Maybe have separate "Summary" and "Description" boxes?
