Fix hibernate resuming from a swapfile

The kernel implementation works just fine, but initramfs-tools needs
a few changes to account for hibernate resuming from a swapfile.
* Look for the swap header at the resume offset specified on kernel cmd line
* Write directly to /sys/power/resume rather than rely on uswpswp.
