Changes for 1.4.0-3

Per our discussion on IRC, here are some changes that address a number of the open bugs in pam. I've uploaded these. Note that it looks like you didn't push the change to 1.4.0-2. If you do push that I'll be happy to rebase. I believe that with the exception of a debian/changelog whitespace issue, I did correctly incorporate the affect of these changes.
