OpenLDAP upgrade notes for bookworm

The slapd 2.5 upgrade is likely to be messy for some users. Any feedback on the text would be welcome. The key points are:
- anyone using (at least) the mentioned modules (bdb, hdb, shell, ppolicy, argon2) will not have a working slapd service after upgrading
- mdb is fully supported in buster and bullseye, bdb/hdb users should migrate BEFORE upgrading to bookworm
- slapd.README.Debian has step-by-step instructions for fixing the upgrade for the known cases. (Is the filename the best pointer for this? Would a link to the live file on sources.debian.org be better?)
