Add some information into doc/README and .htaccess files

I made cosmetic changes in doc/README and added missing information about some scripts, files and directories. In the .htaccess.deriv file empty description for contents of the apt/ dir is added to remove ambiguity (otherwise sources.list.d has the same description as sources.list).wrong_descriptions
