Fix some issues reported by lintian

• Use correct machine-readable copyright file URI. (out-of-date-copyright-format-uri)
• Bump debhelper from old 11 to 13. (package-uses-old-debhelper-compat-version)
• Set debhelper-compat version in Build-Depends. (uses-debhelper-compat-file)
• Set upstream metadata fields: Bug-Database, Bug-Submit, Repository-Browse. (upstream-metadata-file-is-missing, upstream-metadata-missing-bug-tracking)
• Drop unnecessary dependency on dh-autoreconf.
• Fix day-of-week for changelog entry 0.22-4. (debian-changelog-has-wrong-day-of-week)
• Update standards version to 4.6.1, no changes needed. (out-of-date-standards-version)
• Avoid explicitly specifying -Wl,--as-needed linker flag. (debian-rules-uses-as-needed-linker-flag)
