clean up remainders of /usr-merge

Helmut Grohne requested to merge helmutg/debootstrap:cleanup-merged-usr into master

debootstrap accumulated quite a bit of code to support merged-/usr over the years and it changed every now and then. Now that trixie and later handle these matters themselves, it is time to clean up the remainders and restrict the extra logic to non-default cases and discard dead code.
