final menu entry additions (Accessibility)

this is the last of those commits submitted in a previous bootloader related MR, then removed from it for later resubmission. some review was done. some tweaking was done subsequently.

expands accessibility install entries to full set of Debian v10 install disks, having looked at a newer one since the 2015 based change now.

  • full set of speech-synth entries
  • addition of dark contrast entries

grouped into submenus, though flatter than originally, partly due to total quantity of entries, partly because allows use of common hotkeys, which may be particulalry helpful to users of the speech synth entries.

a few tiny adjustments accompany the main change just to get this stuff finished off.

a small backwards compatibility hack is found in the last commit to allow for the file to be renamed along with the label change, which felt right to do, but you may perhaps disagree.

the initial commits here were perhaps reasonable to have pushed, but i felt that was not within the spirit of the agreed permissions so i held back. i hope that the handful of commits pushed directly thus far are all acceptable. :)
