Fix some issues reported by lintian

• Refer to common license file for Apache-2.0. (copyright-does-not-refer-to-common-license-file, copyright-not-using-common-license-for-apache2)
• Bump debhelper from old 12 to 13. (package-uses-old-debhelper-compat-version)
• Set upstream metadata fields: Bug-Database, Bug-Submit, Repository, Repository-Browse. (upstream-metadata-file-is-missing, upstream-metadata-missing-bug-tracking, upstream-metadata-missing-repository)
• Update standards version to 4.6.2, no changes needed. (out-of-date-standards-version)
