Refresh patches, copyright and metadata

Changes best described by the commit logs. I've bundled them together because they are individually so small, to avoid fiddly MRs, but if any is controversial then just shout and I can isolate it!

  • Add d/upstream/metadata with 'new' github repo
    • debian/upstream/metadata | 5 +++++
  • Record MD5 files as supposedly 'public domain'
    • debian/copyright | 17 +++++++++++++++++
  • License my and by default any new patches as per upstream
    • debian/copyright | 8 ++++++++
  • Normalise copyright declaration formatting
    • debian/copyright | 19 ++++++++++---------
  • Reflect changed licence in current upstream
    • debian/copyright | 28 +++++++++++-----------------
  • Mark Debian-specific options patch as not needing forwarding
    • debian/patches/debian-defaults-for-params-h.patch | 1 +
  • Update foreground mode patch following upstream feedback
    • debian/patches/add-foreground-option.patch | 13 +++++++------
